The deadline for submitting an application for the 2012-13 session is June 1, 2012.

Criteria

To be considered for this program, you must meet the following requirements:

• Diagnosed with hemophilia A or B (all severities)
• Between the ages of 18 and 24
• Able to self-infuse your factor concentrate and have access to factor for the duration of conference dates (no factor or infusion assistance can be provided during the program)
• Proficient in written and spoken English
• Able to attend all program sessions
• Able to travel unaccompanied and with appropriate visa and/or passport
• Able to fulfill all aspects of the program's application process

Application Process

To apply for the next session, please fill out the application and submit it to application@stepupreachout.net.

Before submitting your application, please confirm that you have included:

• At least one letter of reference from a teacher, care provider, employer or others able to recommend your participation and one letter of support from your local foundation, association or chapter.
• A personal essay, explaining in your own words your interest in joining the Step Up Reach Out program
• The completed application form

If you cannot email your letter(s) of reference, please fax them to +1 (713) 500-8364 (USA fax number). Please clearly indicate your name, so the fax can be paired with your emailed application.
