Your complete application needs to include the following:

• At least one letter of reference from a teacher, care provider, employer or others able to recommend your participation.
• A personal essay, explaining in your own words your interest in joining the Step Up Reach Out program.
• The application form

To submit an application, please click application for a Word Document version of the application. After completing it, save the document on your computer and email your application as an attachment to the email address below. Please also attach the letter(s) of reference and your essay. Please be sure to type the word "application" as your email's subject.

If you cannot e-mail your letter(s) of reference, you can also fax this document to +1 (713) 500-8364 (US fax number). Please clearly indicate your name, so the fax can be linked to your e-mailed application. We plan on posting a more convenient application upload interface as soon as possible. Thanks and good luck!
